Registration, Health Forms, and Requirements
The health center is open from 8am to 4pm every day school is in session, including during the summer.  Most services are provided at no charge, and no student is turned away for being unable to pay.

Students must be enrolled and have a consent form signed by a parent or guardian on file for many services.  State law allows students to receive mental health and other confidential services (like pregnancy or STD testing) without a consent form on file.
A physical examination is required by law for all students entering: preschool, kindergarten, 6th grade, and 9th grade; and for students transferring into the school district in any grade.

The examination must have been completed within one year prior to entry into the grade and be turned in to the school within 30 days of enrolling.
