Parent or Guardian Consent

Parents or guardians need to complete and sign a consent form so their student can receive services at the SBHC.
State law allows adolescents to receive confidential services (like pregnancy and STD testing) and treatment for mental or behavioral health issues (such as depression, grief, or anger management) without a consent form on file. Students are always encouraged to talk to their parents or guardians about important health issues.
